Take it a piece at a time... there are several parts, and I'm not sure I got them all right, but I tried.
Begins in Dm, 3 opening riffs in unison, verses in C#m (down a half-step?), piano outro, (the loooong one) is in C.
A beautifully crafted song, IMO. I tried. If it's your dream song, you'll learn it in no time. ;)
You'll no doubt notice the clumsiness during the piano outro. Learn it, make it yours, the chords were just really a bit much (a b!tch!) for me to get together. :DR

Thank you. Greenmachine said he'd wanted to play the electric version of Layla forever, and I couldn't get the file (WMM) to work for him, so I "Tubed" it. Now I'm waiting to hear him play it....
